Patient Centered Care
Patient centered care involves integrating patients’ needs and preferences into the delivery of care. It includes a shared decision‐making communication that encourages patients to take part in decisions of their care. We collaborate and encourage a self‐management and patient education with resources that seeks to strengthen our patients’ knowledge and skills to better empower them in their care process.
Patient Engagement
Identifying and closing care gaps is crucial for patient-centered care. The resources to support identifying and closing gaps in care include effective care coordination, regular screenings and check-ups, health timelines, effective patient education and appointment follow-up protocols.
Patient Navigation
We make better patient-centered care with effective navigation. Patient navigation addresses any potential barriers to care experienced by individual patients. Health disparities are differences in the incidence, prevalence, mortality, and burden of diseases or other adverse health conditions that exist among any patient populations. When members need any care, referring to the right provider has a major impact on how much it will cost, the quality, outcomes of their care and especially in addressing their unique needs.
